Common questions about scheduling and what to expect

What happens after I submit this form?

Our team reviews your intake form within 24 hours and contacts you by phone or email to confirm your appointment. The intake information helps us identify your primary complaint and begin mapping the likely Primary Constraint before your first visit, so the initial evaluation can move efficiently from history to neurologic examination. Each episode follows the Care Track Model — a structured progression from intake through discharge.

What should I bring to my first appointment?

Bring any prior imaging (MRI, CT), previous clinical notes, and a list of current medications or supplements. These are helpful context but not required — our evaluation identifies the Primary Constraint through neurologic examination, independent of prior workups. Baseline Capacity Markers are established at the first visit and serve as the reference point for all subsequent progress tracking.

How long is the first visit?

Initial evaluations typically run 60 to 90 minutes. We conduct a comprehensive neurologic examination to identify the Primary Constraint, review your history in detail, and establish baseline Capacity Markers across all relevant domains. This allows us to design a Care Track specific to your presentation, with Readiness Gating criteria defined before the second visit.

Is my information kept confidential?

Yes. Your information is transmitted securely and protected under HIPAA. We do not share your personal or health information with third parties. Your Capacity Markers and clinical findings are maintained in our Clinical Outcome Registry, which is used solely to guide your Care Track and support longitudinal outcome measurement.

Why does PPC treat one primary complaint per episode?

Each episode of care is structured around a single Primary Constraint — the main limiter keeping you stuck — because treating multiple complaints simultaneously prevents accurate attribution of progress to any specific intervention. When you address two complaints at once and improve, you cannot know which Care Track produced the change, which system was actually limiting recovery, or whether Readiness Gating was satisfied for each domain independently. One primary complaint per episode is not a limitation of scope — it is the structural requirement for Constraint-Based Medicine to produce reliable, measurable, and durable outcomes. If you present with multiple complaints, the most functionally limiting one is addressed first. Subsequent complaints are addressed in separate, sequenced episodes once the primary constraint has been resolved and Durability confirmed.
